I understand that it is my responsibility to pick up the materials and return them to the STEM Resource Center at the end of my borrowing period, unless otherwise agreed upon by both parties. 

I understand that if the materials borrowed from the STEM Resource Center are damaged or are not returned, I will be responsible for all costs to repair or replace these items. 

I understand that it is my responsibility to provide both an email AND a current phone number during the account creation process. By agreeing to and providing this information, the STEM Resource Center will be able to contact you in an efficient manner if there are any questions or concerns during the checkout process and the items you are borrowing.
